In collaboration with campus partners, Student Health and Counseling Services offers a series of free flu shot clinics at a variety of campus locations for all students, faculty, and staff throughout the Autumn Quarter. Learn more about the dates and locations of these clinics.

If you received a flu shot at one of our flu clinics and need a copy of your flu form, please fill out this form and email it to mhscsas@maxhealth.com. To expedite the request, make sure you include the date and location of the flu clinic attended. Please allow at least a one week turn around on all requests.
